Lots of little goodies.

Made clone() no longer final, per Michael Skells.

Made Document have removeContent(PI,Comment) per my mail.  Also
removed addContent(Element).

Changed Document Javadocs to use iterator intstead of get(i),
per Guy Nirpaz.

Made getAdditionalNamespaces() return EMPTY_LIST if none, 
per Bob McWhirter.

Simplified SAXBuilder exception handling to not doubly or
triply reproduce root cause error messages.

Updated TODO to reflect these changes.
