Major updates to DOMBuilder:
* It now keeps xmlns namespaces declaration location
* It now relies on the parser to handle namespaces
 (necessary for importing a document that has nodes moved around)

Removed leftover debug stmt from OracleV1DOMAdapter (no one complained,
looks like no one is using it, I just found it in a grep)

Changed XercesDOMAdapter to turn namespace handling on.  Also reformatted
the file to fit 80 cols.
