That is nasty. I really hate NamespaceStack. I wish I had a better 
idea for how to handle what it does. Maybe we could use the SAX 
NamespaceSupport class instead?

In any case, if there's no easy way to get rid of this parameter, 
then we should just delete or make private all the DOMOutputter 
methods that output anything other than a Document.
